Agreement Summary
This document constitutes a legally binding agreement between you ("Volunteer") and WalkTogetherCare ("WTC"). Please review the provisions below carefully before completing your digital execution on the volunteer application form.
1. Operational Scope & Affiliation
I acknowledge that I am volunteering directly with WalkTogetherCare ("WTC"). WTC is solely responsible for organizing, directing, and coordinating all volunteer activities. While I may indicate an affiliation with a community or faith partner (e.g., Reality Church, First Lutheran Church), I understand that such partners do not direct, manage, or supervise WTC volunteer activities.
2. Assumption of Risk
I understand that volunteering (including, but not limited to, lawn maintenance, meal delivery, physical labor, and transportation) involves inherent risks of personal injury, illness, property damage, or death. I knowingly and freely assume all such risks, both known and unknown.
3. Waiver and Release of Liability
I hereby release, waive, discharge, and hold harmless WalkTogetherCare, its directors, officers, employees, agents, and affiliated partner organizations (including any selected community/church partners) from any and all liability, claims, demands, or causes of action arising out of negligence, personal injury, illness, or property damage sustained during my participation in WTC programs.
4. Emergency Medical Treatment
I authorize WalkTogetherCare to secure emergency medical treatment on my behalf if I am injured or incapacitated, and I agree that I am financially responsible for any medical expenses incurred.
5. Media Release
I grant WTC permission to use photos, videos, or recordings of my volunteer participation for promotional, educational, or organizational purposes.
Electronic Signature & Audit Trail Notice
When you submit your Volunteer Application, your typed legal signature, explicit checkbox consent, UTC timestamp, network IP address, browser user-agent, and a cryptographic SHA-256 fingerprint are recorded into the official WalkTogetherCare Legal Audit Log. A flattened 1-page PDF certificate is archived to maintain an immutable compliance record under the Electronic Signatures in Global and National Commerce Act (ESIGN).
